At Randolph Primary Care, we always put our patients first. However, to help us assist you to the best of our ability, there are a few things you can do.
- Please arrive at least 15 minutes early for your appointment to ensure that you have the time necessary to fill out any paperwork and to speak with our front desk staff.
- Please provide us with at least 24-hours advance notice if you need to cancel your appointment. This allows us to open up our schedule for other last-minute appointments.
- Please bring your insurance information along with a payment method to your appointment.
Prior to your visit with us, we invite you to fill out our patient forms. These forms can be downloaded to your computer and printed at home. By filling these out in advance, you’ll save a significant amount of time during your visit.
Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
